ImPACT Passport is a free iPhone or Android concussion care app that helps you stay connected to your concussion care. Store your unique 12-digit code assigned after completing an ImPACT, ImPACT Pediatric, ImPACT Quick Test, or Cognitive Impairment Screener (CIS) or Balance, and securely share it with your healthcare provider when needed.
ImPACT Passport also hosts the Balance assessment, allowing healthcare providers to remotely administer FDA-cleared balance testing through the app. In addition, you can record concussion symptoms and find local healthcare providers, making ImPACT Passport a convenient tool for supporting recovery and ongoing care.
